Eligible employees who have completed one year of service and have worked 1,250 hours in the preceding 12 months may request leave without pay for family and/or medical reasons, with job protection. If you need to request FMLA, please complete the Leave Request Form (PDF) and forward to HR 30 days prior to your expected leave. If you are not able to provide a 30 day notice, please complete the form and forward to HR upon your first notice that you will need leave.
